 You will need to fill out one entry form for each piece you submit. This can be paper-clipped to the front of the entry, or taped to the front of the CD case if your entry is audio or video. If your entry is a blog or blog post, you will find a space on the form to fill in the address. Download the entry form as a PDF. Download the entry form as a Microsoft Word Document you can fill out by typing. Each entry must be accompanied by a check for $5 for the first entry, $3 for each additional entry. For multiple entries from one family, the fees can be combined in one check. Each entry does need its own entry form, though. Make checks payable to Sherene Silverberg. Mail entries to:
